Media Recording & Use Waiver

I acknowledge that iElite Performance Lab may photograph, film, and audio record the athlete during training sessions, evaluations, showcases, and other iElite activities. I grant iElite Performance Lab permission to use those photographs, videos, and recordings for coaching feedback, athlete development, and promotional purposes on the iElite website, the official iElite Instagram page, and other iElite social media and marketing channels. This permission is granted free of charge and does not transfer ownership of any of the athlete’s own material. For athletes under 18, this agreement is made by the parent or legal guardian listed above. Permission may be withdrawn in writing at any time by emailing iElitePerformancelab@gmail.com.

Athletes under 18: a parent or legal guardian must agree on the athlete’s behalf.
